The Clinical Case Manager - Treatment Foster Care, is responsible for providing support to children and families by serving as an advocate for youth, a liaison to other human services and educational agencies, and a co-facilitator of mental health treatment services. Provide ongoing support of youth placed in foster and adoptive homes. Communicate with families, legal systems, schools, mental health providers, probation and social service agencies and provide court testimony as requested. Maintain appropriate documentation and files to support the services that are provided to families.
Supervise the placement and ongoing support of youth placed in foster and adoptive homes assigned to their caseload. Work effectively with and engage biological and adoptive families in the youth’s treatment process. Complete paperwork and compile documentation required by Woodbourne Center and governing/accrediting agencies; maintain current files on all youth in placement. Facilitate quarterly treatment planning meetings and develop an individual treatment plan that provides on-going assessment and diagnosis. Complete the licensing application and training process for assigned new foster parents. Participate in staff and supervision meetings, and required Woodbourne Center in-service meetings. Understands and supports the Woodbourne Center and Nexus philosophy and mission.
Must currently have an active LGSW or LCSW license. Valid driver’s license required and maintain auto insurance that passes corporate insurance eligibility standards. Must maintain CPR/First Aid Certification. Must be able to walk, sit, stand, crouch, twist, bend, reach overhead, maneuver stairs and stretch in a manner conducive to the execution of daily activities for a social worker visiting clients in their homes. Must be able to spend a considerable amount of time driving to client locations and transporting clients as needed using one’s own vehicle.
